As committed in the 2025 National Service Plan, the HSE commissioned an independent review of the Physician Assistant role with the aim of making recommendations regarding the governance of the role, scope of practice, and appropriate and safe deployment in Ireland. The HSE paused the recruitment of Physician Assistants pending the outcome of the review. The HSE sent the completed report to me and I expect that the HSE will publish it shortly. Following publication of the report, the HSE will work towards the implementation of the review’s recommendations and the establishment of an appropriate grade code for the role.

I expect that patient safety in service delivery be prioritised in relation to any HSE workforce roles, including Physician Assistant. My officials and I continue to engage with colleagues in the HSE on the Physician Assistant role in Ireland, including the implementation of the review’s recommendations.

With regard to the details of the agreement between the HSE and the RCSI for funding student fees, I have asked the HSE to respond to the deputy directly, as soon as possible.

It is disappointing that SIPTU and UNITE have notified the HSE of their decision to proceed with industrial action, up to and including strike action, in the National Ambulance Service. This issue has been the subject of extensive engagement over the last two years, including full use of the State’s industrial relations machinery through the Workplace Relations Commission and the Labour Court.

The HSE, the Department of Health and Department of Public Expenditure accepted both the WRC proposals and the subsequent Labour Court Recommendation in full — demonstrating good faith and a willingness to resolve the dispute. Both unions recommended those proposals to their members, but the proposals were rejected by their members in a ballot.

The Government Departments and the HSE have since demonstrated a clear willingness to re-engage to find a resolution. The unions declined that offer and instead moved immediately to a ballot for industrial action. The HSE, DoH and DPER remain fully committed to the dispute-resolution mechanisms under the Public Service Agreement 2024–2026. A ballot for industrial action at this point is unwarranted and risks unnecessary disruption to emergency care. At all times, the priority must be the safety of patients and the continuity of emergency services, and I strongly urge all parties to re-engage constructively and use the established State mechanisms to bring this matter to resolution in the interest of the public.

In the proposed revision of the EU Pharmaceutical legislation the European Commission has proposed further strengthening measures and specific initiatives, such as the introduction of the Union list of critical medicines and the requirement for medicine suppliers to have in place shortage prevention plans (‘SPP’). Such measures are anticipated to further improve supply chain resilience and promote continuity of supply. My department, together with the Health Products Regulatory Authority (HPRA), is committed to continuing its active involvement at European level to maximise the effectiveness of this work and translate it into tangible national improvements.

Several initiatives have been taken over recent years at a European level to improve security of medicine supply and address supply chain vulnerabilities that are common to multiple markets. These include an expansion of the mandate of the European Medicines Agency (EMA) to optimise the management and prevention of shortages at European level.

Addressing medicines shortages is complex and requires input from multiple stakeholders. A Medicines Shortage Framework (MSF) has been in place since 2018 when the Department requested that the HPRA assume responsibility for its coordination. Since its inception, the framework has been of enormous benefit nationally. It has served to avoid the occurrence of potential medicine shortages and, where they have occurred, ensured appropriate mitigation is in place.
