Deputy David Cullinane asked the Minister for Health the steps her Department is taking to assess and mitigate the impact of the US most-favoured-nation pharmaceutical pricing policy and related trade tariffs on patient access to innovative medicines and the competitiveness of the Irish pharmaceutical sector; the status of the planned inter-departmental or sectoral taskforce established to address these global pricing shifts; when this taskforce is expected to convene; the stakeholders that will be represented; and if she will make a statement on the matter. The Government?is committed to providing timely access to new and innovative medicines to all patients.

The State has made considerable investments in new medicines in recent years, with annual expenditure on medicines now approximately €4 billion. This represents almost one euro of every eight spent by the State on healthcare.

Supported by 158 million euros of funding in Budgets 2021-2025, the State delivered access to 250 new medicines. Budget 2026 allocated 217 million euro of additional investment in medicines, including 30 million euro in funding for new medicines. This has, as of mid-June 2026, delivered access to 26 new medicines.

The Year 1 cost of a new drug comes from the allocation for new medicines, but it should be noted that once these medicines are approved for reimbursement the full cost of providing them can reach multiples of this initial cost as their uptake increases.

The sustainability of State medicine expenditure must be protected. A central contributor to this is the Framework Agreements on the Supply and Pricing of Medicines (FASPMs) agreed between State and pharmaceutical industry since the 1970s. New Agreements were signed in March 2026 for the next four years. These Agreements provide stability to the medicines reimbursement market for the State and for Industry, thus providing certainty and stability to the supply and pricing of medicines in Ireland.

These Agreements include a commitment to a stepwise programme of change to achieve reimbursement decisions within the statutory 180-day period by Q1, 2029. As part of structured process towards achieving a 180-day timeline for reimbursement decisions by Q1 2029, an end-to-end review of the pricing and reimbursement system, will be conducted, starting this year.

Achieving this shared goal, requires the pharmaceutical industry and the State to work together, through timely assessment, reasonable pricing and fully prepared health technology assessment dossiers being submitted by a company to the HSE.

Ireland continues to collaborate with European colleagues to support efforts to enhance medicines access, availability, and affordability in Europe.

Foetal Alcohol Spectrum Disorder (FASD) is a prevalent, yet preventable, neuro-developmental disorder. FASD is caused by prenatal alcohol exposure and can lead to lifelong challenges with learning, behaviour, health, and social skills. Depending on their needs children, young adults, and families living with FASD may need support from different services, including primary care, disability services, and mental health care.

As Minister for Health, I am committed to ensuring this Government delivers the appropriate services for each person with FASD in line with their needs.

Under the Health (Pricing and Supply of Medical Goods) Act 2013, the Health Service Executive (HSE) has statutory responsibility for decisions on the pricing and reimbursement of medicines. The HSE is assessing the pricing and reimbursement application for omaveloxolone (Skyclarys®) in line with the criteria detailed in legislation.

The HSE received a commercial proposal on the 27th May 2026 from the company Biogen(Idec)Ireland Limited.

Following the conclusion of commercial negotiations, applications are progressed via the Drugs Group who will make a recommendation to the HSE Senior Leadership Team who have final decision making authority on whether to reimburse a medicine.

No decision has been made by the HSE Senior Leadership Team, which has final statutory decision making authority for the reimbursement of medicines in line with the Health (Pricing and Supply of Medical Goods) Act 2013.

Medicines are brought forward for consideration at Drugs Group in line with the progression of commercial engagements and the receipt of finalised proposals, ensuring an orderly and fair process for all applications.

The HSE will continue to assess and progress the application as efficiently as possible. As this application remains under consideration with the HSE and the HSE cannot make any comment on possible outcomes from the ongoing process.

Eligibility for a Medical Card is primarily based on a financial assessment which is conducted by the HSE in accordance with the Health Act 1970 (as amended). The HSE assesses each medical card application on a qualifying financial threshold. This is the amount of money that an individual can earn a week and still qualify for a card. It is specific to the individual’s own financial circumstances. Current medical card income thresholds can be considered generally in terms of cohorts aged 70 or older, and under 70 years of age, with separate thresholds and assessment criteria for each.

With regard to the estimated cost of the Deputy's proposals, the information sought is not readily available and will need to be determined. Officials from my Department will be in further contact with the Deputy directly with relevant information as soon as available.

Under the Health (Pricing and Supply of Medical Goods) Act 2013, the Health Service Executive (HSE) has statutory responsibility for decisions on the pricing and reimbursement of medicines. The HSE assessed the pricing and reimbursement application for givinostat (Duvyzat®) in line with the criteria detailed in legislation.

The commercial proposal along with other relevant information was progressed to the Drugs Group on 9th June 2026, who assessed all the information, including submissions by patient representative organisations, and prepared a recommendation for the HSE Senior Leadership Team.

On foot of the Drugs Group recommendation, the HSE Senior Management Team, which holds final decision-making authority on the reimbursement of a medicine in line with the Health (Pricing and Supply of Medical Goods) Act 2013, has made a positive reimbursement decision and commissioned a Managed Access Protocol for givinostat.

All efforts will be made to expedite the implementation of the Managed Access Protocol and to ensure that an adequate stock of the medicine is available and delivered to patients as quickly as possible.

Our national mental health policy Sharing the Vision 2020-2030 builds on well-established cross-sectoral links to develop a range of mental health supports to ensure timely and appropriate care in an appropriate setting. This includes improving interlinkages between the specialist HSE National Forensic Mental Health Service (NFMHS) and the judicial system.

It is acknowledged that demand for forensic mental health services has grown in recent years, particularly in the last five years. This includes demand for inpatient beds at the Central Mental Hospital (CMH) at Portrane. The HSE actively manages the CMH waiting list for admission and works closely with the Irish Prison Service to prioritise access for those with the greatest clinical need.

The new National Forensic Mental Health Service (NFMHS) 170-bed complex at Portrane includes a new 130-bed CMH, a 30-bed Intensive Care Rehabilitation Unit (ICRU) and a 10-bed forensic Child and Adolescent Mental Health Service Unit. The CMH relocated to its new state-of-the-art purpose-built health facility in November 2022, transferring 93 patients to the new NFMHS campus. It was always intended that the new facility at Portrane would become operational on a phased basis.

At present, there are 114 operational beds in the CMH, which represents a 20% increase in bed capacity compared to the old hospital in Dundrum.

I secured €2.1 million under Budget 2025 to recruit the staff necessary to open the remaining beds in the CMH, thus ensuring continuity of care for patients there. In addition, I allocated €2.8m in new funding under Budget 2026 for an additional 30 Whole Time Equivalent (WTE) posts to open the first phase of 10 beds in the new ICRU. Opening these beds, will allow patients to be moved more efficiently through the various levels of high-quality supports offered at Portrane, which will in turn allow for further admissions across the wider NFMHS.

When these 10 ICRU beds come on stream towards the end of this year, they will bring the total number of operational beds on the NFMHS campus to 140. This represents an increase of 45 beds or 47% when compared to Dundrum. The new funding provided this year will bring the total NFMHS staff complement to approximately 580 WTEs.

While the Department of Health and the HSE fully appreciates the increasing demands on the prison service, it is important to stress that the NFMHS campus in Portrane is a specialist tertiary healthcare facility. This facility is approved for the purposes of the Mental Health Act 2001 and the Criminal Law (Insanity) Act 2006. Every effort will continue to be made by the health sector to help address acknowledged waiting list pressures to access the NFMHS overall, including on-going recruitment of specialist health professionals.

I will continue to work closely with the HSE to ensure that all aspects of our mental health care programme are improved over this year and beyond to develop capacity in the system to meet future demand. This includes consideration under Estimates 2027 to open the remaining beds at Portrane.

Sexual health is a priority for the Department of Health. The National Sexual Health Strategy, 2025-2035 (NSHS) and National Sexual Health Action Plan, 2025-2038 (NSHAP), commit to developing a Model of Care for sexual health.

The Model of Care for Sexual Health is being developed by the HSE Sexual Health Programme. It is our understanding that it will be published at some point in Q3-4, 2026
