Deputy Pádraig Rice asked the Minister for Health the oversight that exists to ensure that National Incident Management System (NIMS) reports filed by clinical and technical staff are properly investigated; the mechanisms, if any, to prevent a report being voided at hospital management level without independent review; and if she will make a statement on the matter.
